Instructions to Arrange Transportation:
1.) Leave a message stating you need transportation in
the following format:
Example: "This is "Joe," I am
a client of Direct/Client Services and my Case Manager is__________.
I am calling to request transportation from "departure location" to
"arrival destination." I have an 11am appoinment.
A representative from MCATF Client Services will
return call to get additional information and setup a reminder call
1 day before your trip.
2.) Continue to call until someone calls back to
arrange transportation. In the event we can arrange payment of
your taxi bill to the doctor / hospital. We both have to work to
assure that you do not miss your ride.
* Mileage, figured at the current state rate, will be deducted from the
client's (the one you are transporting) yearly funds. (For
example, a transportation volunteer drives a client from their home in
downtown Bloomington to HIHAC in Peoria for an exam and then returns
with client to starting point [no stopovers] ... if IL State Mileage
Rate is $.34/per mile and you travel a total of 80.02 miles round-trip,
your reimbursement would be ($.34 x 80.02 = 27.2068) for that particular
trip. Mileage is turned-in monthly and reimbursed the following
month.
